You can feel safe in knowing that we are a NDIS registered provider. NDIS registered providers are regulated by the NDIS Quality and Safeguards Commission. All providers (registered or unregistered) must adhere to the NDIS Code of Conduct. Support Coordination
At Lead the Way, your Support Coordinator will help you to understand how to utilise your NDIS plan and access support to achieve your goals.
​
Your Support Coordinator will assist you to understand and use your NDIS plan by connecting you with NDIS providers of your choosing and build your confidence and skills to use and coordinate your supports independently - if this is your goal!
Together with one of our Support Coordinators, you can develop a support plan on how to best achieve your goals. We will connect you with service providers in the community, both NDIS and mainstream or other government services who are the right fit for you. Your Support Coordinator will work with you to coordinate the delivery of services and supports by connecting you with service providers of your choosing, help you to understand and access funding and resources, and coordinating care across different providers and organisations and ensure service agreements and bookings are undertaken as planned.
Our Support Coordination team is here to make your life easier. We will help you understand service agreements, start your plan, make the most out of your funded supports, delivering supports that can help you to achieve goals and most importantly help you in preparing for annual NDIS plan reviews by collecting he required documents and evidence required to assist you with achieving your goals.
Specialist Support Coordination
At Lead the Way, your Specialist Support Coordinator provides complex case coordination to reduce barriers to implementing your NDIS plan and mitigate risks associated with situational crisis preventing you from achieving your goals.
A Specialist Support Coordinator is like that of a Support Coordinator with the addition of providing complex case coordination. For example, you may be facing homelessness, require alternative accommodation options, or are involved with other organisations such as the justice system, mental health services or child protection. The Specialist Support Coordinator will support you to coordinate and /or link you to NDIS and state funded services and assist you in navigating the multiple interfacing sectors to support you in achieving your goals.
At Lead the Way, our Specialist Support Coordinators are experienced in providing complex case coordination to mitigate crisis and interface between multiple sectors, your support coordinator can ensure all your services and supports are working effectively and cohesively together to ensure you achieve your ambitions.
Recovery Coaching
At Lead the Way, your Recovery Coach will help you access supports best suited for your disability and to understand how to utilise your NDIS plan and access support to achieve your goals.

A Psychosocial Recovery Coach (recovery coach) is an option for participants with psychosocial disabilities who need assistance navigating the complexities of daily life. The goal is to enhance participants’ social and economic independence and engagement through capacity-building support.
​
Recovery coaches will empower participants to take a more active role in managing their lives and will work in collaboration with other services to help achieve these outcomes. They will partner with participants, their families, caregivers, and various services to design, plan, and implement a recovery plan, while also coordinating NDIS and other supports.
Justice System
For individuals involved in the justice system who may be incarcerated and need release planning, Specialist Support Coordination can assist in navigating the forensic landscape and connecting you with various services and resources, including both mainstream and NDIS-funded options.

This support may include linking you to accommodation and service providers that help you stay safe in the community while meeting any conditions imposed by the justice system, which might be difficult due to your circumstances.
Hospital Discharge
For individuals who have undergone unplanned hospitalization and need discharge planning to transition back to their preferred accommodation in the community, your Specialist Support Coordinator will coordinate all NDIS-funded inpatient treatment teams and community-based care providers. This collaboration will help ensure a smooth discharge process and ongoing support to prevent rehospitalization and reduce risks in the community after discharge.
Exploring Housing Options
This support is designed for individuals facing significant barriers in finding suitable accommodation that meets their unique needs. You may require assistance in relocating between different service providers or locations due to changes in your situation.

Additionally, if you are experiencing homelessness and need help securing mainstream or NDIS-funded accommodation, your Specialist Support Coordinator will work with you to explore housing options that best align with your specific requirements.
