What Is a Support Coordinator?
Under the NDIS, a Support Coordinator communicates with service providers, organises and connects you to supports funded within your plan, makes sure budgets are followed, assists you with your plan reviews, and can build your capacity to learn how to manage your plan by yourself.
At Lead the Way, we all know that having an NDIS plan can be overwhelming at the best of times, which is why our Support Coordinators are there to assist you with implementing your NDIS plan.

A key function of your Support Coordinator’s role is knowing about appropriate services in your local community to match your specific needs. Your Support Coordinator will assist you in selecting between the different providers to identify a choose the right ones for you! Using their knowledge of the NDIS services and systems, your Support Coordinator will ensure that providers meet your Quality and Safeguards requirements.
Your Support Coordinator will also ensure you are getting the most out of your NDIS plan and ensure that your services are supporting you to achieve your goals. Are you facing obstacles that need adjustment to help you achieve your goals? Do you have new goals or want to try something new? Have your circumstances or needs changed?

These are the important questions for anyone with a NDIS plan, particularly when it’s time for a plan review. Our Support Coordinators can assist you to answer these questions and support you to gather the necessary evidence for your review as well as assisting you to negotiate changes to funding within your plan. With Lead the Way you don’t have to go through process alone.

After your Support Coordinator has learned about your circumstances, they will help you plan for any changes and unexpected events. These could include unplanned hospital stays, needing to move houses, or changes in the availability of support from your family or service providers. If any of these would affect your NDIS services, your Support Coordinator will support you to develop a contingency plan to ensure you can work through these changes and stay on track with your goals.

Most importantly, your Support Coordinator can build your capacity to eventually manage your own plan – if this is one of your aspirations. For example, they can show you how to use the NDIS systems, set up supports, and ensure you are keeping to your budget. This will empower you to advocate for yourself and make important decisions in pursuit of your goals.

For individuals who may be incarcerated and require release planning, the Specialist Support Coordinator can help navigate the forensic space and provide access to your chosen NDIS services and accommodation services ready for release.


Hospital Discharge Planning
The Specialist Support Coordinator will help individuals who are transitioning from hospital care back to their chosen accommodation in the community by coordinating NDIS and mainstream funded services that may be necessary for their recovery and ongoing care post discharge.


Exploring Housing Options
A Specialist Support Coordinator is able to assist participants looking to explore alternative accommodation options that may me better suited to their disability needs, or for those who require to source accommodation as a sense of urgency due to homelessness.
